The practice welcomes all patients residing in the practice area.

To register you need to complete this form 2020 New Registration Form and provide photo identification (ID), as well as proof of address.  These could be a passport and/or driving licence as well as a utility bill and/or bank statement.

This form can be used for Stanford Medical Centre, Islingword Road and Cockcroft Surgery (University of Brighton students)

Patients from outside the UK will also need to complete the NHS England registration form in addition to the above

GMS1 form to be printed Please ensure you complete the rear section titled 'supplementary questions'

Once you have completed the registration form please forward the required proof of photographic ID & Proof of address to
